2024 Guide to the Interstate Medical Licensure Compact (IMLC)

The Interstate Medical Licensure Compact (IMLC) simplifies the process for obtaining a medical license in 35 U.S. states and one U.S. territory. The IMLC creates a faster pathway for licensure, it also adopts a uniform and stringent standard for licensure and affirms that the practice of medicine occurs where the patient is located at the time of physician-patient encounter. A physician that holds their primary license in one of the participating states is eligible for the IMLC, which provides expedited licensure in any of the other states at a discounted rate.

*Alabama, Connecticut, and Vermont are not “States of Principle Licensure”. You cannot enter the compact through AL, CT, or VT licenses, however you can obtain a license in these states through the compact.

IMLC Qualifications:

Two types of eligibility requirements, SPL eligibility and general requirements, providers must meet both types.

SPL:
Must hold a full, unrestricted medical license in a state that is a member of the Compact - this is known as your State of Principle License (SPL)

In addition, at least ONE of the requirements below must be met:
- Primary residence is in the SPL
- At least 25% of your practice of medicine occurs in the SPL
- Your employer is located in the SPL
- You use the SPL as your state of residence for U.S. Federal Income Tax purposes

In order to remain active with the IMLC, you must maintain your SPL status at all times. You may re-designate your SPL to mother state after you have received your initial qualification, as long as the state is a participant.

General Eligibility:
- Must have graduated from an accredited medical school, or a school listed in the International Medical Education Directory
- Must have successfully completed ACGME or AOA accredited graduate medical education (GME)
- Must have passed each component of the USMLE, COMLEX-USA, or equivalent in no more tan three attempts for each component
- Must hold a current specialty certification or time-unlimited certification by an ABMS or AOABOS board

In addition, you must:
- Not have any history of disciplinary actions toward your medical license
- Not have any criminal history
- Not have any history of controlled substance actions toward your medical license
- Not currently be under investigation
